MEMBERSHIP TERMS
Individuals may become a member ("Member") of Nutrimaps.com ("Nutrimaps.com" or "Site") by joining the Site. Membership is free.This site is not recommended for use by women who are pregnant or lactating without direct medical supervision. If you are pregnant or lactating, it is recommended that you seek the advice of your healthcare provider or dietitian for advice on determining calorie and nutrient needs.
MEMBER OBLIGATIONS
a. Abide by the "Terms And Conditions" posted on the Site, which are incorporated by reference and constitute a part of Nutrimaps.com's agreement with you;
b. Maintain the security of your user identification, password and other confidential information relating to your Nutrimaps.com membership account;
c. Be responsible for all charges resulting from your use of the Site including any unauthorized uses, unless you take steps to prevent its further occurrence by changing your password; and
d. Update your personal information, including e-mail address and billing information.HEALTHCARE PROVIDER MEMBER OBLIGATIONS
a. Abide by the "Terms and Conditions" posted on the Site, which are incorporated by reference and constitute a part of Nutrimaps' agreement with you;
b. Acknowledge and Agree that you are a licensed or certified healthcare provider ;
c. Maintain the security of your user identification, password, and other confidential information relating to your Nutrimaps.com membership account ;
d. Maintain the privacy and confidentiality of all members that a healthcare provider-patient relationship has been established with;
e. Update your personal information, including email address and facility name.
CANCELLATION
A member may cancel anytime as follows:
- By e-mail: Send an e-mail to support@nutrimaps.com indicating in the message your intent to cancel. Be sure to include your first and last name and user name. Please note that we will not accept or open e-mail communications with attachments.
- By mail: Send a brief letter, including your first and last name, user name and e-mail address, and mail it to: eHealth Software Solutions, Inc. PO Box 345 Devault, PA 19432 Attn: Member Services.
- By telephone: Call Nutrimaps.com at 610-296-9091 and ask for the Nutrimaps.com coordinator.
Nutrimaps.com may cancel your membership pursuant to the Terms and Conditions.
